With our recent president in the minds of many, I was reminded by your title of the fact that terms such as “moron" and “idiot” started out as scientific, medical terms.

As they became widely known by the public at large, their connotation, their intent, morphed into derogatory territory.

The definition of any word is a statement of how it is used by society in general, at the time. This changes, as language evolves.
